Medical Records & Certificates

 
 
Requests for medical records by patients require a filled out and signed DD Form 2870 and proper identification. Please fill out the Authorization for Disclosure of Medical or Dental Information DD Form 2870 on our forms page Patients can present the request in person or fax the form along with a copy of their identification.

Medical Correspondence will copy all available electronic and hard copy file medical records. Copied medical records can be faxed, mailed (paper copies or on a CD) or sent through a secure e-mail service within 30 days.

Medical records with no activity after two years are transferred and retired to the National Personnel Records Center (NPRC), Saint Louis, Missouri. To request these records, please go to www.archives.gov

Civilian medical organizations requesting patient medical records may use their own release forms in lieu of DD Form 2870.

Insurance companies must mail their request with a search fee check for $20.80. Requests will be processed and sent within 30 days. Some requests may be sent to the Madigan Legal Office for review prior to release.

Public defender requests will be processed immediately at no charge.

For radiology images, Medical Correspondence can only send written results or notes of the taken images and not the actual images. Please contact the Madigan Radiology Film Library at 253-968-2226 or fax the request to 253-968-2024. To complete the request for images, the Film Library will also need a filled out and signed DD Form 2870 and proper identification.

Defense Enrollment and Eligibility Reporting System (DEERS)

The Patient Administration Division (CAC/DEERS Office) enrolls newborns into DEERS. The CAC/DEERS office is located in Room 1-94-04, first floor Patient Admission & Dispositions (PAD) office in the Hospital Tower.

**New Changes** Effective immediately, a Hospital Proof of Birth/Certificate of Live Birth document is no longer valid to enroll a child, including a stepchild or pre-adoptive child to a Sponsor's DEERS record or to process a newborn placeholder record already in DEERS. Per Human Resource Command Project Office, Hospital Proof of Birth is not supported by statutory requirements specified under 32 CFR, Part 161, Tables 2 and 3 of Subpart D. Please present the original or certified copy of the State Birth Certificate along with the child’s Social Security Card to be enrolled into DEERS.
