Madigan’s Patient-Centered Medical Home Primary Care Clinics are Army Medical Homes with an interdisciplinary approach to delivering evidence-based, comprehensive primary care– coordinating care delivered outside of the primary care setting and proactively engaging patients as partners in health.

The Army Medical Home is designed around one core principle: putting patients first. The Army Medical Home is Army Medicine’s gateway to influence the “life space” where patients make decisions on the key determinants of health and wellness–sleep, activity, and nutrition (that is, the Performance Triad).

Each patient will partner with a team of healthcare providers – physicians, nurses, behavioral health, clinical pharmacists, physical therapy, and case management professionals to develop a comprehensive, personal healthcare plan. This team will form a partnership with you to provide improved access, coordinate needed services, answer your questions, and ensure that you get the care you need. Your Medical Home team will remain the same as long as they continue to work within your medical home. This will improve your continuity of care and allow you to develop a closer partnership with your team. The Army Medical Home model has been shown to improve patients’ overall health, resulting in fewer emergency and urgent care visits decreased hospital admissions, shorter inpatient stays, and fewer inpatient readmissions among other positive results.

To transition from a “healthcare system” to a System for Health, moving away from episodic care to a standardized, long-term model that achieves optimal health outcomes. The Army Medical Home model will facilitate the transformation to a System for Health and be the primary care means for providing patient-centered comprehensive care to patients. The desired end state is for all patients to receive coordinated, comprehensive care guided by the Army Medical Home. The key to this coordination is improved communication using face-to-face and technological methods such as MHS Video Connect, MHS GENESIS Patient Portal and the Nurse Advice Line, strengthened by a firm patient-provider relationship.

Adult Walk-In Services

The Madigan-Puyallup Community Medical Home provides the following walk-in services for its established patients, Monday through Friday 9:00 a.m. - 3:00 p.m. (closed from Noon - 1:00 p.m. daily)
  • Immunizations
  • Pregnancy testing
  • Infant Weight Checks
  • Suture Removal (if sutures were placed at the Madigan Puyallup Medical Home, otherwise, it will require an appointment with your PCM)


Who can be seen at this location?

The Madigan-Puyallup is open to military family members and a limited number of retirees and their families with active duty dependent and military retiree beneficiary status living in, and around the Puyallup, Graham, Spanaway and Frederickson WA area.
  • The Madigan Main Outpatient Pharmacy Drive-Thru. Call the automated refill line at 253-968-2999 and Choose option 1 for the Madigan Drive Through. Please contact your triage nurse if you are having trouble using the automated refill line.
  • Express Scripts: Though there is a co-pay, this is another method for you to receive your medications. Please contact the triage nurse for your provider to discuss. Please be advised, this method can take several days to process.
  • Retail Pharmacy: You can move your current prescription to a TRICARE retail pharmacy. Please contact the pharmacy of your choice, and give them our phone number of 253-477-5084. A nominal copay may apply. *Please note, Target/CVS does not take Tricare insurance.
 We also would like to encourage our patients to utilize the MHS GENESIS Patient Portal’s Secure Messaging feature to message their healthcare team. The MHS GENESIS Patient Portal puts you in control of your health care. It gives you secure access to your electronic medical record 24/7 and allows you to reach out  to your health care team when and where it’s convenient for you. GPS tends to get patients lost. Make your way to Meridian East which is the main road through Puyallup and the clinic is in a large shopping complex. Enter Sunrise Shopping Center complex from 156th Meridian East, a Chase Bank is on the corner, follow the road straight back thru 4 stop signs to the T in the road, clinic on left, Valley Bank on the right. Our Parking lot will be directly in front of you but you will have to turn right or left and go 25 meters to enter it!
