Case Management
What is Case Management?
Case management is a process in which a registered nurse or social worker works with you and your health care team to assess, plan, coordinate, implement, monitor, and evaluate the options and services required to meet your unique health care needs. Case management helps coordinate the care among members of the health care team. The case manager is a patient advocate working with providers, ancillary health staff, and the family to address areas of concern, with informed decision making, and help patients reach their best possible level of wellness.
Why would I need Case Management?
The need for case management services is determined through a collaboration between you, your family or caregiver, your health care team, and your provider. The collaboration will include a comprehensive assessment of your health, psycho-social needs and use of health care services and resources.
