What is Case Management?

Case management is a process in which a Registered Nurse or Social Worker works with you and your healthcare team to assess, plan, coordinate, implement, monitor, and evaluate the options and services required to meet your unique healthcare needs. Case management helps coordinate the care among members of the health care team. The case manager is a patient advocate working with providers, ancillary health staff, and the family to address areas of concern, with informed decision making, and help patients reach their best possible level of wellness.

Why would I need Case Management?

The need for case management services is determined through a collaboration between you, your family or caregiver, your healthcare team, and your provider.  The collaboration will include a comprehensive assessment of your health, psycho-social needs and use of health care services and resources.

What Can A Case Manager Do for Me?

  • A Case Manager can Partner with your healthcare team and help coordinate your care with multiple providers and specialties.
  • Explain options for care and assist you in making informed health care decisions.
  • Assist you in taking control of your health by establishing and achieving personal health goals.
  • Provide education, support, and advocacy.
  • Help you and your significant other make informed health care decisions.
  • Assist with discharge planning.
  • Assist with transitioning your health care to another facility.

Do I qualify for Case Management?

You may qualify for case management if you have:
  • A serious health condition(s) or a terminal health condition.
  • Several health conditions being treated by many specialties.
  • A need for additional support during a critical period or need for assistance navigating the health care system.
  • Difficulty following your plan of care to stabilize your health condition.

How can I receive case management services?

If you feel like you would benefit from Case Management assistance, talk with your health care provider.   Your provider will place a medical referral request and then one of our Case Managers will contact you.
