How to Receive Services
Your primary care provider will conduct an initial evaluation, at which time they may place a referral to CAFBHS through your electronic medical record. Once the referral from your primary care provider is received, CAFBHS staff will contact you to discuss your treatment options. It is important that you have updated contact information in your medical record.
*Please note that CAFBHS does not see walk-ins. If you require a same-day appointment you may be seen at your patient centered medical home or you may seek emergency care at your nearest emergency room.
