About

Madigan’s Patient Centered Medical Home Primary Care Clinics are Army Medical Homes with an interdisciplinary approach to delivering evidence-based, comprehensive primary care– coordinating care delivered outside of the primary care setting and proactively engaging patients as partners in health.

The Army Medical Home is designed around one core principle: putting patients first. The Army Medical Home is Army Medicine’s gateway to influence the “life space” where patients make decisions on the key determinants of health and wellness–sleep, activity, and nutrition (that is, the Performance Triad).

Each patient will partner with a team of health care providers – physicians, nurses, behavioral health, clinical pharmacists, physical therapy and case management professionals to develop a comprehensive, personal health care plan. This team will form a partnership with you to provide improved access, coordinate needed services, answer your questions, and ensure that you get the care you need. Your Medical Home team will remain the same as long as they continue to work within your medical home. This will improve your continuity of care and allow you to develop a closer partnership with your team. The Army Medical Home model has been shown to improve patients’ overall health, resulting in fewer emergency and urgent care visits, decreased hospital admissions, shorter inpatient stays and fewer inpatient readmissions among other positive results.

To transition from a “health care system” to a System for Health, moving away from episodic care to a standardized, long-term model that achieves optimal health outcomes. The Army Medical Home model will facilitate the transformation to a System for Health and be the primary care means for providing patient-centered comprehensive care to patients. The desired end state is for all patients to receive coordinated, comprehensive care guided by the Army Medical Home. Key to this coordination is improved communication using face-to-face and technological methods such as the MHS GENESIS Patient Portal and the Military Health System Nurse Advice Line, strengthened by a firm patient-provider relationship.

Direct Appointment Booking - MHS GENESIS Patient Portal (Starting Jan. 23, 2026)

McChord Family Medicine assigned patients ONLY: 

The MHS GENESIS Patient Portal will enable Direct Appointment booking by clinic - starting January 23, 2026. This will allow patients assigned to McChord Family Medicine Clinic to directly schedule an appointment from their MHS GENESIS Patient Portal with their assigned PCM or another Provider in their assigned Primary Care clinic. *All Defense Enrollment Eligibility Reporting System (DEERS) eligible beneficiaries can use our prescription services. The pharmacy can fill original prescriptions from civilian or military sources, and transfer refills from other military installations as long as the pharmacy carries the medication. Patients may also use TRICARE Pharmacy Home Delivery or a TRICARE civilian network pharmacy.

Adult Walk-in Services

The McChord Family Medicine Clinic provides these walk-in services for enrolled adult patients Monday - Friday, 7:30 a.m. - 3 p.m.

  • Immunizations (Air-Force Active-duty service members only for walk-in, all other eligible beneficiaries require a scheduled appointment)
  • Pregnancy Test

Medication Disposal

The McChord Family Medicine offers a secure receptacle for the safe disposal of unwanted, unused or expired medications. The blue "medsafe" receptacle is located in the pharmacy waiting area of the clinic. This receptacle is a one-way drop door open to accept medications during regular business hours.

Accepted medications for disposal in the medsafe include prescription and over-the counter medicines to include controlled-substance medications, vitamins, medicated lotions/ointments, liquid medication in leak-proof containers, and transdermal skin patches. Items that are not suitable for deposit in the medsafe are needles (sharps), illegal drugs, aerosol spray cans (to include aerosol inhalers), and chemicals or hazardous waste.
