Who Needs a Sports Physical?
👦 Students aged 5 and up who plan to join school sports
🏃♀️ Those who haven’t had a sports physical in the past two years
🩺 Students with recent changes in health status
When Are These Checks Available?
📅 Weekdays: Starting July 1st, we have a dedicated provider available for sports physicals.
📅 Saturday Clinics: Special dates on July 20th and August 17th.
How to Book an Appointment:
📞 Call the Tricare Appointment Line at 800-404-4506 and specify you need a sports physical for your child.
What to Bring:
📋 If it's the initial sports physical or recent health change (within the past two years):
Bring the pre-filled sports physical paperwork provided by your child’s school.
📋 If a sports physical was completed in the last two years with no health change:
No appointment is needed! Just provide the pre-filled forms to your child’s care clinic, and their primary care provider will complete them based on the most recent examination.
Submit forms via the patient portal or drop them off at the primary care clinic.
